Does Creatine Cause Hair Loss? What the Research Says

Creatine is one of the most researched supplements in sports nutrition, and many people taking GLP-1 medications are also using creatine to preserve muscle mass during weight loss. But a persistent question keeps coming up: does creatine cause hair loss?

The Short Answer

The evidence linking creatine directly to hair loss is limited and based primarily on a single small study. However the concern is not entirely without basis.

The 2009 Study Everyone Cites

A 2009 study published in Clinical Journal of Sport Medicine followed college-aged rugby players who took creatine supplements for three weeks. The study found that creatine supplementation was associated with an increase in dihydrotestosterone (DHT) levels — a hormone known to contribute to male pattern baldness in people who are genetically predisposed to hair loss.

Importantly the study did not measure actual hair loss. It only measured DHT levels. The jump from elevated DHT to actual hair loss requires a genetic predisposition to androgenetic alopecia.

What is DHT and Why Does it Matter?

DHT is a byproduct of testosterone metabolism. In people genetically sensitive to DHT, it can bind to hair follicles and cause them to shrink over time, eventually leading to hair loss. This is the mechanism behind male pattern baldness and female pattern hair loss.

If you are not genetically predisposed to DHT-related hair loss, elevated DHT from creatine is unlikely to cause noticeable hair thinning.

GLP-1 Medications and Hair Loss

People taking GLP-1 medications like semaglutide or tirzepatide face a separate and more well-documented hair loss risk. Rapid weight loss and caloric restriction can trigger a condition called telogen effluvium — a temporary form of hair shedding that typically begins 2 to 3 months after starting treatment.

For people taking both creatine and GLP-1 medications, the combination of potential DHT elevation from creatine and telogen effluvium from rapid weight loss may increase hair loss risk in genetically susceptible individuals.
